Supported states include CA, CO, CT, DE, IL, IA, KY, MN, MT, NE, NH, UT, VA. The form requires an email address plus a facial photograph — Clearview cannot search their database by name, only by image. For access requests a government-ID photo is also required (id_required set true; the engine must redact before upload). Authorized agents are conditionally accepted (must be authorized to act on behalf of the state resident). Confirmation arrives via email link.
