The opt-out web form at go.kindsight.io/privacy-opt-out requires first name, last name, email, mailing address, city, postal code, and country. The OneTrust portal is also linked from the privacy policy for CCPA access/deletion requests. Authorized agents are accepted per CCPA per privacy policy ('Authorized Agents may make requests under the CCPA on behalf of California Consumers by filling out the form').
