The form (contact-us.aspx) requires name, email, phone, address, a reason selection ('CCPA Request' for CA residents or 'Opt-Out Request' for others), subject set to 'Do Not Sell', plus upload of a government-issued photo ID (driver's license). Requests via email or phone are redirected to the online form. After submission, broker sends an email verification confirming opt-out. Authorized agents accepted with comparable identity verification documentation. Broker self-asserts it does not sell/share data but is still registered as a CCPA data broker in CA, OR, and TX. Processing takes up to 45 days. Legal basis mapped to CCPA-1798.120 (opt-out of sale) per the broker's own CCPA framing. id_required=true because government ID upload is mandatory for the request to be processed.
